Heritage Nanny Community Living and Support (HNCLAS) provides support to individuals who are experiencing developmental, mental health, behavioural, and emotional challenges. HNCLAS offers a program to individuals with Complex Needs and another program for individuals with Disabilities. The program is designed to provide them with the necessary skills and well-being to attain a greater level of independence within the community.
Position Summary
The Mental Health Programs Consultant is a critical member of an interdisciplinary team with responsibility to contribute to the development, implementation, and evaluation of treatment plans with individuals diagnosed with complex needs/developmental disability. This role is accountable to the Manager and/or Director.

Education: Bachelor's degree in in Human Services or Bachelor of Social Work
Experience: 1 year to less than 2 years
On site: Work must be completed at the physical location. There is no option to work remotely.
Responsibilities:
- Maintaining accurate client records and preparing detailed reports to track client progress and outcomes.
- Perform administrative tasks, produce reports, recruit and hire staff, train staff, and help maintain high standards of care for the clients.
- Update individualized care plans tailored to the client’s care needs.
- Scheduling multidisciplinary team meetings that foster staff engagement and collaboration,
- Offer guidance on trauma-informed care practices and crisis intervention techniques.
- Promote ongoing professional development and capacity-building among care teams.
- Maintain accurate documentation of consultations and client progress.
- Working collaboratively with teams of professionals to deliver integrated, person-centered care.
- Ensure that mental health services are effectively integrated into the day-to-day care of clients.
- Participate in team meetings to monitor progress.
- Advocating for additional resources or specialized services as needed for high-risk clients.
- Assisting with crisis intervention and de-escalation strategies as needed.
- Implement mental health programming, such as individual or group therapy and life skills sessions.
- Producing detailed reports on client outcomes and service delivery, which inform continuous improvement.
- Present the views of an association or organization to members, the media and the general public
- Write articles meant for the general public about wellness issues.
- Maintain, update, and manage health information databases.
- Monitor health care programs.
- Present the views of an association or organization to members, the media, and the general public.
- Organizing or attending conferences and meetings
- Conduct interviews
- Conduct literature reviews.
- Act as liaison within the organization between staff and management around wellness issues.
- Design and implement health and wellness projects or programs.
- Develop or update policies, standards, and guidelines.
- Coordinate workshops.
- Coordinate multidisciplinary projects.
- Deliver presentations at conferences, workshops, or symposia.
- Monitor and compile research results.
- Offer mentoring.

Qualifications & Skills
Bachelor's degree in Human Services or Bachelor of Social Work
One to two years related experience
Demonstrated ability to work effectively as part of a team
Computer literacy with Microsoft software
